For the second year in a row, the Lapeer County Health Department, Home Health Program has been named as one of the Top 500 performing home health agencies in the United States. Data gathered by the Centers for Medicare and Medicaid Services ranked almost seven thousand agencies nationwide in three areas:
•Quality of care measurements;
•Quality improvement activities;
Through hard work, an experienced staff, honest work ethics, and their commitment to the community, they have proven their dedication to quality and performance in this highly competitive environment.
The Lapeer County Health Department Home Health Program has also received the Michigan Award of Excellence for decreasing hospitalization. They are doing their part to help Michigan's financial situation by protecting necessary services in Lapeer County and maximizing Medicare health care dollars.
The Lapeer County Health Department Home Health Program has been providing services to residents of the county since 1969. It is the only agency in Lapeer County that gives back to the people any remaining funds to help supplement health related services provided by the Health Department. These include much needed services such as help for pregnant women, infants, and children; immunizations for children; community education classes on health related topics; and home care programs for seniors.
The Home Health Program assists clients in remaining safe and independent in their homes. Services include: RN, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker, and Home Health Aides. Remember to ask for them by name when your doctor orders home care services-The Lapeer County Health Department Home Health Program.
